When You Don't Know What Comes Next

One of the most difficult parts of caring for a loved one is knowing where to begin.

From Everyday Support to Skilled Home Health

Every person does not need the same level of care.

Some individuals need assistance with everyday activities while remaining as independent as possible. Others may require medically necessary skilled services following an illness, hospitalization, surgery, rehabilitation, or change in health condition.

YCIOP has two distinct service branches designed to address these different needs.

YCIOP Home Care — Non-Medical Support

Non-medical home care can provide practical assistance that helps an individual remain safe, comfortable, engaged, and supported in daily life.

Depending on the individual's needs and the services authorized, support may include:

  • Personal care assistance
  • Companionship
  • Homemaker services
  • Meal preparation
  • Respite support
  • Transportation assistance
  • Grocery shopping and errands
  • Medication pickup and reminders
  • Light housekeeping
  • Support with daily routines

The goal is to provide meaningful assistance while respecting the individual's independence, dignity, preferences, and routines.

YCIOP Medical — Skilled Home Health

When an individual meets applicable requirements and needs medically necessary skilled care, home health may become an important part of the recovery or care journey.

Skilled home health services may include:

  • Skilled nursing
  • Physical therapy
  • Occupational therapy
  • Speech therapy
  • Medical social services
  • Home health aide services
  • Care coordination
  • Patient and caregiver education

Services are provided according to the individual's needs, applicable requirements, physician or authorized practitioner orders when required, and an established plan of care.
