Chronic Disease Management

Parkinson's Disease Support

YCIOP provides coordinated care for individuals living with Parkinson's disease as their needs change over time. Our team monitors symptoms and functional changes, supports medication management and safety, and coordinates with healthcare providers and therapy services to help maintain comfort, mobility, and quality of life.

Neurological Care Coordination

Patients living with neurological conditions may experience changes in mobility, strength, communication, cognition, or daily functioning. YCIOP provides individualized skilled care, ongoing assessment, patient and family education, and coordination with physicians and other healthcare professionals to address each patient's unique needs.

Compassionate End-of-Life Support

During life's final stages, compassionate care and meaningful support become especially important. YCIOP provides comfort-focused skilled care, symptom observation, emotional support, and close communication with the patient, family, and healthcare team while honoring the patient's wishes, dignity, and individual needs.

Palliative Support

Palliative support focuses on helping individuals with serious or complex illnesses live as comfortably and fully as possible. YCIOP coordinates care to support symptom management, patient and family education, communication among healthcare providers, and care decisions that reflect the patient's goals and preferences.

Diabetes Care Coordination

YCIOP Home Health helps patients manage diabetes safely at home through skilled monitoring, medication support and education, blood glucose observation, and attention to nutrition and foot care. We coordinate with the patient's physician and healthcare team to help identify concerns early and support a care plan that promotes stability and confidence.

Stroke Rehabilitation & Recovery

Recovery after a stroke often requires ongoing support and coordination. YCIOP works with patients, families, physicians, and therapy providers to support rehabilitation goals, monitor changes in condition, encourage safe mobility, and help patients work toward the highest possible level of independence.
